Student Discipline Sheet for Mr. Heidenreich’s Math Classes

I. School-Wide Discipline Plan:

The student will:

a. Comply with all requests from any staff member – certified or classified.

b.
Use designated areas of the building and grounds for the activity for which they were designed for.

c.
Use all school equipment and facilities appropriately.

d.
Avoid inappropriate conduct such as fighting, unlawful acts, verbal abuse, etc.

e.
Attend classes.

f.
No food or drinks in the wings.

g.
No cell phones or any type of electronic devices are allowed on campus.

II. Classroom Rules:

The student will:

a. Not be talking while the teacher is talking.

1. Every day there will be a short assignment on the overhead for you to do when you enter the room. I will expect you to work immediately on that assignment when the bell rings. I call this assignment board works.

2. Any time I am talking in front of the room, I expect you to be quiet and paying attention.

3. I will always expect you to raise your hand if you have a question or you want to sharpen your pencil or whatever. This means stay in your seat unless I give you permission to get out of your seat.

b. Follow all directions.
1. This means the first time I tell you to do something then I expect you to do it.

2. I will expect you to work by yourselves and quietly on any assignment that I give you during class. There will be days when we will be working in groups of twos at your table. But, I expect you and your partner to be working together and not with other people.

c. Bring all materials needed for this class every day.
1. This means that you have to bring your book every day. I do not have enough books for you not to bring your book. This also means you should have a pencil or pen, notebook, graph paper, ruler, compass, calculator, and protractor every day.

d. Always be seated in your assigned seat.
1. I will have a seating for you each month. You will be expected to be in the seat that I have assigned you every day.

e. Not to be tardy.
1. A tardy means that you are not seated in your assigned seat when the bell rings. I do not care what your reason is for being tardy unless you have a note form another teacher or administrator. You will be counted tardy even if there was a fight or any other reason that you consider a good reason for being late.

2. I expect you to be on time for my class. You are taking up my time and all the other students’ time when you are late.

3. You are tardy if you are late to my class less than 5 minutes. After 5 minutes you are absent or truant to my class. Truant means it was an unexcused absence and you cannot make up any work for that day.

4. But, if you are late to my class you will have the following consequence:
- First tardy- verbal warning. I will just say to you that is your first tardy and it is a warning.
- Second tardy – You will receive a copy of the tardy policy to be signed by you and your parent/guardian and brought back to me the next day. If it is not brought back to me you will automatically have a lunch detention and I will call your parent or guardian to let them know that you did not follow through and that you also have two tardies.
- Third tardy – Lunch detention will be assigned for the next day. I will give you a lunch detention slip and you will take it to the administrator that is watching lunch and the administrator will assign you a duty to do during lunch. The slip must be signed by the administrator and brought back to me.
- Fourth tardy and anymore after that – I will write a referral to the administrator and they will handle the appropriate discipline for you.

f. Remain on task in class.

1. When I give you time in class to work on an assignment then that is what I expect you to do.

g. No lining up by the door before the bell rings.

h. No food or drinks in the classroom.

i. No headphones, cell phones or any other type of electronic device in the classroom. If I hear a phone or any other type of electronic devices then Iwill confiscate them and give them to the security guards and you will have to go and do an after school detention to get them back.

III. Failure to Follow the Above Rules Will Result in the Following Action:

a. Verbal warning.

b
. Call home to parent/guardian.

c.
Lunch detention.

d.
Student sent out of the classroom for the day.

e.
A referral sent to the administrator and they will give out the appropriate discipline.

IV. Academic Expectations:

a.
Make-up work for assignments due to an excused absence is your responsibility. You must have an admit to get into my class. Even if you forgot your note from your parent/guardian you still must get an admit that says you have a note due truancy and then you have three days to bring your note and clear it up. You will have one day for each day you are gone to make up assignments.

b.
If you miss a test due to an excused absence you will take the test the next time you come to class. If you are truant to my class on the day we have a test you will get a zero on the test. Any other day that you are truant you will receive a zero on any assignment for that day.

V. Extra Credit:

a.
In your notebook if you do extra notes or do a good job on your assignments for that week then you will get extra credit. So another words
every Friday when you turn in your notebooks you could get extra credit. Also, you are allowed to use your notebooks on the tests and quizzes so the better notes that you take the better you will do on the tests.
