FIELD TRIP CONSENT FORM

The purpose of an educational field trip is to provide students with information and experiences that are not available within a classroom. When properly integrated into the plan for instruction, a field trip can provide a valuable learning opportunity.

In keeping with policies of the Vancouver School District it is necessary to secure the written permission of the parent or guardian prior to student’s participation in a field trip.

When signed, this consent form is your grant of authority for your child to participate in the designated field trip.
