FIELD TRIP CONSENT FORM
The purpose of an educational field trip is to provide students with information and experiences that are not available within the classroom. When properly integrated into the plan for instruction, a field trip can provide a valuable learning opportunity.
In keeping with the policies of the Vancouver School District it is necessary to secure the written permission of the parent or guardian prior to student's participation in a field trip.
When signed, this consent form is your grant of authority for your child to participate in the designated field trip.
THE UNDERSIGNED HEREBY GIVES PERMISSION AND AUTHORIZES

I UNDERSTAND THAT THE TRIP, WHETHER MADE IN A SCHOOL BUS, PUBLIC TRANSPORTATION OR WALKING WILL BE SUPERVISED BY SCHOOL AUTHORITIES AND THAT EVERY EFFORT WILL BE MADE TO INSURE STUDENT SAFETY.
