VANCOUVER PUBLIC SCHOOL DISTRICT FIELD TRIP CONSENT FORM

The purpose of an educational field trip is to provide students with information and experiences that are not available within the classroom. When properly integrated into the plan for instruction, a field trip can provide a valuable learning opportunity.

In keeping with the policies of the Vancouver School District it is necessary to secure the written permission of the parent or guardian prior to student's participation in a field trip.

When signed, this consent form is your grant of authority for your child to participate in the designated field trip.

TRANSPORTATION IS PROVIDED BY DR. QUESTAD AND VOLUNTEERS WHO HAVE SIGNED THE VSD DRIVER STATEMENT
All participants must be present at the Skyview flagpole
no later than 7:20 am Sat. morning, 12/14.

All participants will be dropped off at the Skyview flagpole at approximately 7:15 PM Saturday evening.
It is required that participants have pre-arranged transportation from Skyview at that time.

ALL STUDENTS MUST TRAVEL UNDER THE SUPERVISION OF THE DEBATE ADVISOR.
NO STUDENT MAY PARTICIPATE AT A DEBATE TOURNAMENT OUTSIDE THE VANCOUVER SCHOOL DISTRICT
USING TRANSPORTATION IN AN UNAUTHORIZED VEHICLE.

I UNDERSTAND THAT THE TRIP, WHETHER MADE IN A SCHOOL BUS, PUBLIC TRANSPORTATION OR WALKING WILL BE SUPERVISED BY SCHOOL AUTHORITIES AND THAT EVERY EFFORT WILL BE MADE TO INSURE STUDENT SAFETY.
